Welcome to Chemeketa Community College and Storm Athletics. Below you will find the required athletic requirements to be able to participate in your sport. These must be completed before you participate in any activities with the program.

All first year incoming student athletes will need a new collegiate physical. All other student athletes will need a new physical if their last one was more than two years from there last competition. To receive access to the physical & Immunization form needed you must complete your Sportsware account and download the form from your account.

We have attached the Chemeketa Athletics requirements needed to participate in your program. Please make sure to thoroughly go through the materials attached.
